Apparently, Yeong-jo has the plague. The Noron faction want to return to the palace so he can be properly treated, but he can’t be moved. Then they realize that if he dies, San is automatically king, and it’s all over for them. Sir Choi sends word to the Queen via Jeong. San sits with an unconscious Yeong-jo.
At the palace, it is the 9th day of the 9th month, so court ladies are allowed out of the palace to see the autumn leaves. The queen leads Lady Hye (initially apprehensive due to Yeong-jo’s absence), Lady Hyeoui and various court ladies out for their special day. They arrive at a Buddhist temple, only to find Princess Hwaran praying there. She’s been praying for 7 days for Hyeoui to get pregnant, says her serving lady.
They see Princess, who says she heard Lady Hye wanted Hyeoui to get pregnant, so she’s just doing her part. She presents her prayer beads to Hyeoui. Lady Hye, suspicious, asks how Princess knew she wanted Hyeoui to get pregnant. Princess says she always is concerned about the well-being of the royal family, so she keeps up with the news, and is praised by the Queen for her care.
Later, Lady Hye discusses with Hyeoui that Princess knew they’d come by that way and set up “this commotion” so it would get back to Yeong-jo. She cautions Hyeoui not to fall for her tricks. They have to do everything they can to keep her out of the palace. Elsewhere, the Queen remarks that Princess must be pretty desperate to pull this stunt. She advises Princess to grab her chance to win the king’s heart when it comes, and work slowly to build up his trust until then. Then the Queen will help her.
At that moment, the Queen’s lady reports that Yeong-jo has fallen ill on his trip. Q later rants about the lack of responsibility evidenced by everyone on the trip, who let him get sick. At her quarters, Ladies Hye and Hyeoui discuss the situation with their serving ladies. Princess forces her ladies to get ready for a trip to the palace even though she’s not allowed to go there; she figures her life is over anyway if San gets to be king.
The Queen receives a letter from Jeong. She declares the Norons to be idiots, and gives her lady a letter to deliver to Sir Choi. She says San has to die if Yeong-jo dies; rather, she says, he has to die before Yeong-jo does. She scolds Princess when she shows up, and declares that San will never become king.
San walks among the plague victims. He discusses with the royal doctor that young adults are fine if they take medicine, but children and the elderly die. They agree it’s because these people “lack energy.” They’re waiting for special herbal medicines from Qing (China). Until then, they’ll make do with their own medicines and recruit doctors from the area to try to contain the plague. San’s advisors tell him he should go to the district office to get away from the sick people, but he wants to stay awhile. Then he hears that Yeong-jo is conscious and wants to see him.
Yeong-jo orders San to return to Court, to guard it until he can return himself. He tells San this is his duty as crown prince. Elsewhere, Jeong and Sir Choi plot to destroy San at the Queen’s order, even if this means to attack him directly. Later, Jeong tells his man that he plans to use someone else’s hand to pull his quarry out of the fire rather than the rebels they planted in the police force.
San and his advisors and guards discuss how to outsmart the Norons. Hong wants him to send a decoy procession down one of the two roads to the capital, then sneak on the other road to town himself. San objects to his guards being attacked in his place. The guards want to do it. San overrules everyone and says they’ll all return in one procession. Outside, the advisors discuss it, but Sir Chae says they’ll never get San to change his mind. Hong seems to be plotting something. Hong tells the second head guard to deliver a letter to the governor at any cost, that he holds San’s life in his hands. As the man leaves the compound, Hong is startled by Dae-su, who needs a favor.
The Dowahseo members are stuck doing menial tasks like washing dishes. They moan about it. Song tells them they won’t get plague if they are careful about what water they drink (must be cholera), but Cho-bi declares she doesn’t believe that, even the king got sick. Mr. Tak declares that he “needs his rest,” and sponsor Lee pretends to be sick and skulks after him, leaving Cho-bi and Song stuck with all the remaining dishes. Song, having gone to fetch them, finds San getting ready to leave camp. At that moment, Dae-su comes in and finds Cho-bi by herself. She is thrilled until he tells her he’s there for Song.
San gives Song some palace medicine, which she tells him to take. He tells her she must stay well, and that he’s returning to the palace, and holds her hands. Dae-su watches, and does not have a chance to talk to Song himself. The procession leaves. Yeong-jo is told San has left the area.
Next morning, Sir Choi and Jeong discuss San’s possible route. Jeong says he’s ready.
The governor’s men meet San’s group. Hong tells San they can rest there for now. Nam agrees this is good idea, as their horses weren’t even fed and rested enough before they left. The head guard tells Dae-su “it’s time.” Hong advises San to take the longer route to the capital. Nam and Hong tell San to rest while they take care of everything. Nam and Hong give each other the secret nod.
A procession takes the shorter route to the capital, and a spy sees them and orders a guy to attack them leaving no one alive. It turns out this second guy is a brigand, and his fellows come swarming out of the brush to lay in wait. A small group breaks off from the procession, including Dae-su, dressed as San. The rest of the guards return to the real San, presumably. The head guard cautions everyone to be on their toes and tells Dae-su he’ll guard him no matter what, as he’s “the prince” now.
At his camp, San finds out about the deception and is furious. Hong tells him he had no choice. Then San finds out that there were only 8 men in the group, as they needed the rest to guard him. He’s very upset, and realizes Nam was in on it. Nam explains they had to do it and begs San to accept the loyalty of his guards who lay their lives down for him. Then San finds out Dae-su was in the group. He wants to rescue them, but it’s too late---they left an hour before.
The bandits attack the 8 guards. (Looks like they’re tripping the poor horses, instead of using horses trained to fall. Bad form.) Dae-su, shot by an arrow, breaks it off. They try to escape, but they are outflanked. The guards fight valiantly, showing up their excellent training, but the bandits are too many for them. The head guard screams retreat, then goes to Dae-su’s rescue (the focus of the murderous mob due to his disguise). The two are the only ones standing when they are rescued by the other head guard and a battalion of soldiers. The bandits are chased off, but it looks like Dae-su is bleeding to death.

Hyeoui is disturbed to see San’s thoughts seem to be elsewhere. San learns there is a rush on medicine in the capital city and inflation in the prices.

Choi wonders how they’ll face the queen now that San’s taken over at the palace. He decides they need to move Yeong-jo, even if he’s not ready.

Then he learns only three guards survived the attack and that they’ve returned, wounded. San visits a very weak Dae-su in the pharmacy dept.
At his lady friend’s tavern, Uncle receives a nice meal for helping her with repairs. She declares he will be an excellent husband, as good as a eunuch is to his wife. Uncle chokes and stammers, and she thinks he doesn’t know what a eunuch is. At that moment, the head guard comes to tell Uncle about Dae-su. Uncle runs to scold and worry over D-S.
